Enhanced Care Coordinators- “Enrich"

Senior Allied Health and Mental Health Nursing positions available in Melton and Bacchus Marsh.

In 2018, the North Western PHN provided funding to respond to the identified local need of four local government areas of Macedon Ranges, Melton, Moorabool and Sunbury in response to a growing need of support to young people, aged 12-25 years, who are experiencing difficulty with mental ill-health and co-occurring complexity such as alcohol and drug issues. The target group seeks to engage those who are most vulnerable to ongoing and enduring mental health issues and disadvantage.

Orygen, the National Centre of Excellence in Youth Mental Health leads a consortium of partners being Cobaw Community Health and Sunbury Community Health and Djerriwarrh Health services who will employ local teams delivering a combination of face to face, outreach and integrated online moderated programs. They will be supported by Consultant Psychiatrists and in partnership with local primary care and tertiary mental health services to develop a consistent regional service model.

Initial recruitment has been successful, with more opportunities to join the Enrich team at Djerriwarrh Health Services in Melton and Bacchus Marsh. The clinicians will assist in engaging and support young people with complex needs to seek and maintain their treatment within an enhanced primary care setting. This role will work within a range of organisations and health services to respond to the needs of young people with a range of mental health and situational issues. The roles will seek to engage in a community of practice and service development amongst the other three areas.
